CLASS OF 2009...COLLEGE COUNSELING UPDATE


APPLICATION FORMS AND THEIR PATH

 

DEADLINES

  October 1 All secondary school materials and teacher recommendation forms with deadlines prior to December 1 must be given to Ms. Lenger and appropriate teachers no later than this date.

  November 17 All secondary school materials and teacher recommendation forms with deadlines prior to February 1 must be given to Ms. Lenger and appropriate teachers no later than this date.

  December 1 All state university applications, especially for out of state schools, should be mailed by this date for students to receive maximum consideration by the universities in question.

Secondary School Report Form
(can also be called Counselor's Recommendation)
Application

  Give to Ms. Lenger, along with a STAMPED envelope addressed to the college (return address is PHS)
  Turn in ENTIRE, COMPLETED, SIGNED application to Ms. Lenger
  Be sure to fill out the top portion of the the form (name, address, etc.)   Be sure to include a check for the APPLICATION FEE and a STAMPED envelope addressed to the University
  Be sure to SIGN the confidentiality waiver
(if applicable)
  Keep a copy for your records.
    These applications are mailed directly from Pembroke Hill after a transcript has been attached
 
Mid-year Report Form
(i.e., request for Fall Semester grades)
University of California

  Give to Ms. Lenger, along with a STAMPED envelope addressed to the college
  Send completed application directly to the University processing center
  Be sure to fill out the top portion of the the form (name, address, etc.)  Keep a copy for your records.
  Be sure to SIGN the confidentiality waiver
(if applicable)
  Inform Ms. Lenger which campus(es) you applied to within the U/C system
 


Teacher Recommendation Form(s)
   Give to the teacher(s) that you're asking to write for you, along with a stamped envelope addressed to the college
   Be sure to fill out the top portion of the form (name, address, etc.)
   Be sure to SIGN the confidentiality waiver (if applicable)
   Be sure your teacher(s) has (have) a copy of your most recent resume
   Keep your teacher(s) informed regarding your admission status (they now have a vested interest!)

 

The Application (i.e., the student portion)
   TYPE or utilize available software
   If you apply ONLINE, be sure to notify Ms. Lenger so that she can forward a transcript
   Have someone you trust review your ESSAYS (e.g., see your college counselor, Ms. Gordon, or your English teacher)
   Make a COPY for your files.
   Be sure to SIGN the application (some applications require a parent signature as well)
   Be sure to enclose the APPLICATION FEE
   Answer all questions and appropriately (see your college counselor if questions about disciplinary actions concern you)
   Some Early Decision programs require the signature of your college advisor on the Early Decision agreement form

 

On-Line Application
   Complete student portion of application on line
   You will pay the application fee with a credit card
   Print the transcript request form and bring it to Ms. Lenger
   If there is no transcript request form, tell Ms. Lenger that you have applied on line
   Ms. Lenger will forward your transcript to the appropriate school(s)
